This is a quick overview of the vision and dental plans Coda offers for our US-based employees. For a more detailed summary of benefits, please login into SequoiaOne and select Healthcare. You’ll be able to see and compare all the plans offered in your state.

Plans

Dental and Vision plans offer flexibility to see any dentist or specialist in or out of the network.
Costs are lowest when enrollee visits a participating network provider.
No ID cards needed! Simply provide the identifying information requested by the dental office
The accumulators such as deductible and annual benefit maximum reset each calendar year, on January 1st.

Vision

Plan: VSP Vision Premier
Standard plan: Coda covers 100% of premium

Dental

Plan: Guardian Dental Premier Plan and Guardian Dental Standard Plan
Standard plan: Coda covers 100% of premium
Premium Plan: This is an optional plan and considered a buy-up. You can see the exact amount when you are making your elections. The additional amount will be deducted from your paycheck

How to Find an In-Network Provider


To find a dentist in the Guardian Network, follow the steps below:
Hover over “"Connect with Us” then click “Find a provider”
Under “Search for a dentist in your area”, select “PPO” for Plan Type
Enter search criteria

To find a vision provider and search the VSP Network, follow the steps below:
Go to guardiananytime.com
Hover over “"Connect with Us: then click “Find a provider”
Click on “Find a Vision Provider” (located to the left of the “Login/Register” button)
Under the Select Your Vision Network, click “Search VSP”
Enter search criteria
