The goal of this assessment is to develop an agreement on where we are now. We can't hope to make any transformation successful if we don't agree where the starting line is.
Before we start, it's important to agree at what level we are answering these questions. Are we answering for the overall organization, a department, or just one team?
The 4-point scale:
Barely started - There's little to no evidence that the statement is true for us
Early gains - There might be pockets of evidence, but it's not widespread or consistent
Getting there - The statement is true for us, but we still need to improve or be more consistent
Nailing it - The statement defines who we are and how we work
