Help troubleshooting Intro Page error

A prospective merchant is reporting seeing a format error when submitting an application. A screenshot of what they are seeing on their end below. Does any one know what could be driving the error message?

this error happens if the Leadgen form uses the old request format

basically, this looks like old instances of the QUO leadgen form is somehow being loaded and sending the old payload

From "our side" we would likely need to follow up with Quo to ensure the old leadgen code is not being served up somewhere, or isn't being cached at some DNS somewhere
