An Industrial Trainee must be supervised by a Field Supervisor appointed by the company.

Assessment by the field supervisor must be done by using the Confidential Field Supervisor’s Report Form (Online or Print) and Student's Attendance Form (attached below).

Both Forms must be submitted through email or in a sealed envelope directly to ITC.

Student is responsible to ensure that the said form is submitted to the field supervisor at the respective organization.
