Build a Case Index

A Case Index will contain a set of Cases that need support for a vulnerability.

A Case may be generated from;

an entry in a Cohort

a person self-referring and asking to be assessed and supported

a referral from a partner service

a concern raised about a Person or Household

Cases are raised by

the Lead Organisation

other Organisations as directed by the Lead Organisation.

the reason, and source, for raising the case

relevant Risk Factors

an initial situation assessment

a Risk Category

the capacity of Organisations to provide support
