Contract 568312 - Champions for Health: Vaccine Implementation Plan

Section 2: Shelters

Shelters: Providing Critical Vaccination Services to Vulnerable Communities
The "Shelters" chapter of our vaccine implementation plan delves into CFH’s efforts to deliver essential vaccination services to vulnerable individuals experiencing homelessness and residing in various shelter settings. While shelters were not explicitly stated as a target population in the contract, our response to the rising cases of Hepatitis A infections within this community led us to develop and expand our clinic settings in this sector. This chapter provides an overview of our strategies to target this population, the activities of our CFH Homeless Clinic Coordinator, our targeted outreach initiatives, robust reporting and tracking mechanisms, and our ongoing work in mitigating Hepatitis A infections, including the gift card incentive program. Our commitment to protecting the health and well-being of those experiencing homelessness is at the core of our shelter-based clinic endeavors.
