County Data on Hepatitis A Infection Rates:
One of CFH’s key data sources is the county's data on Hepatitis A infection rates. This information provides crucial insights into the prevalence of infectious diseases within the homeless community. In the early stages of CFH’s Hepatitis A Initiative, CFH concentrated outreach efforts in central San Diego, where the majority of Hepatitis A cases were identified. This focused approach allowed CFH to address the immediate needs of this population.
Expanding Outreach County-Wide:
Over time, in response to the evolving situation and guidance from the county, CFH expanded their outreach efforts to encompass all parts of San Diego County. This broader approach ensures that homeless individuals throughout the county have equitable access to vaccination services.
Leveraging Infection Surveillance Data:
In addition to Hepatitis A infection rate data, CFH also draw insights from the county's infection surveillance report. This data enables CFH to track and respond to emerging infectious disease trends within the homeless population.
Reviewing Clinic Event Data:
To further refine CFH outreach efforts, CFH is analyzing clinic event data to identify specific zip codes within HPI (Healthy Places Index) areas where CFH can provide enhanced support to homeless-serving partners. This data-driven approach allows CFH to allocate resources strategically and target their services where they are most needed.

CFH actively seeks out partnerships with organizations like the San Diego Housing Coalition, which may offer incentive opportunities to incentivize vaccination among the homeless population. Our commitment to customized outreach is guided by our unwavering dedication to serving the homeless population effectively. By adapting our strategies, addressing individual needs, and prioritizing accessibility, we strive to make a meaningful impact on the health and well-being of individuals experiencing homelessness in our community.
Section 2.1.3: Vaccination Goals
The objectives and goals CFH set for vaccinating individuals in both sheltered and unsheltered situations are dynamic and adaptable to the prevailing circumstances. At the height of the Hepatitis A infection rates, CFH’s primary goal was to conduct shelter clinics five times a week. This intensive approach aimed to rapidly reach and immunize a significant portion of the homeless population residing in shelters, mitigating the risk of further infections.
As the infection rates began to decrease and stabilize, CFH adjusted our vaccination goals to ensure sustained protection while optimizing resource allocation. We transitioned to conducting at least three shelter clinics per week, maintaining a consistent presence to offer vaccinations, education, and support to shelter residents. These goals are subject to continuous review and refinement based on the evolving needs of the homeless population and guidance from our public health partners.
