To enhance CFH’s focus and effectiveness in serving homeless populations, CFH appointed a dedicated Homeless Clinic Coordinator. This key team member plays a pivotal role in orchestrating CFH’s outreach efforts, ensuring smooth event logistics, and nurturing partnerships with homeless service providers.
The Homeless Clinic Coordinator takes charge of charting and communicating essential event details. They collaborate closely with our Scheduling Coordinators to seamlessly integrate vaccination events into our scheduling platforms, including Connecteam calendars and public-facing event calendars. By maintaining a clear and up-to-date schedule, they ensure that our clinics are easily accessible to individuals experiencing homelessness.
One of the critical responsibilities of the Homeless Clinic Coordinator is overseeing CFH’s gift card incentive program. This involves meticulous review of patient receipts, monthly totals, and the procurement of gift cards. By managing this incentive program, CFH incentivize vaccination participation and improve access to immunization services among our target population.
In addition to these core duties, the Homeless Clinic Coordinator actively engages with the broader homeless community and service providers. They participate in homeless resource work groups, where collaborative efforts are forged to address the unique challenges faced by those experiencing homelessness. Furthermore, the Coordinator conducts site visits to new locations when necessary, ensuring that our services are adaptable and responsive to the evolving needs of this dynamic population.
The Homeless Clinic Coordinator serves as a linchpin in our commitment to delivering accessible, equitable, and effective vaccination services to individuals facing homelessness. Their dedication to event management, partnership cultivation, and the gift card incentive program helps us reach and protect those who are among the most vulnerable in our community.
Section 2.2.2: Community Health Workers
Our dedicated Community Health Workers play a pivotal role in supporting our clinics.
They are not only responsible for fielding new location opportunities but also actively participate in clinics by engaging with the community.
They provide education, assist patients in completing registration forms, and serve as community mirrors, fostering a welcoming environment at our clinics.

Our Data Analyst is instrumental in our efforts to gather and analyze data from our monthly progress reports.
They provide valuable insights into our successes, helping us identify areas of focus and partners with whom we can collaborate more effectively.
