As an alternative to directly referring to partner support Services, the Responsible Organisation may use the information collected during the Assess phase, to suggest a number of relevant services that are available.
This might be offered as a digital self-service facility; some apps have been developed that
take the user through a self-assessment of their Needs

… or this can be a mediated conversation where an agent looks up a Directory of Services and passes the information to the Person.

Use the Directory of Services to list entries that fit to Needs and circumstances.

Follow Up
The person may agree to a follow up contact, some time later, which can be used to capture
which services were accessed
