The SAVVI Concept Model defines ‘Outcome’ as

Outcome
A possible result that can be achieved by addressing the NEEDs of a Person or Household.

Each Need should have one or more associated Outcomes defined.
A set of statuses can be defined to track the progress of a Case as a whole.

Define Outcome for Needs
The Outcomes that could be reported on for each Need are defined e.g.
Child attending school
Domestic abuse has stopped

Define Case Statuses
The status of Cases can be defined, to indicate where in the process each Case is.
The SAVVI Logical Model makes recommendations for Case status
