Closing a Need and Closing a Case

Following new information, such as …
updated Risk Factors
a new Situation Assessment by the Responsible Organisation
a Status Update from a Delivery Organisation
… the Responsible Organisation could close one of the Needs for a Case in the Case Index.

The whole Case may be closed when either
all Needs have been addressed
or
it is no longer relevant e.g. the Person has moved out of the area.
Outcomes are recorded for each Need, and for the Case as a whole.
These updates can be applied individually, or automatically by processing other data.

Steps

Updated Risk Factors

The Lead Organisation may receive an updated set of Risk Factors, which may be used as evidence that a Need is now met.

A New Situation Assessment

The Responsible Organisation may have a follow up contact with the Person and record an updated Situation Assessment, leading to an updated set of Needs. Some or all of the Needs may now be met, or no longer relevant.

Status Update from a Delivery Organisation

Where a Need has been referred, the Delivery Organisation may provide a Status Update to indicate, from which the Need can be updated.

Close a Need

A Need can be closed with a date, and an Outcome added with a description of the new circumstances.

A Case can be closed with a Date, and a final status.
