In this section, we will walk you through the Clinical Records functionality and features.

The patient’s clinical record is a confidential record that should be kept by the clinician and the patient itself. This section is more directed towards the doctor. But. other than the doctors in charge of the patient, the staff can still view the overview of the patient data, periodontal chart, and the treatment plan of the patient.

Periodontal Chart


A periodontal chart is a tool used by dentists and hygienists to record and track the health of a patient's periodontal (gum) tissue. The chart includes measurements of the depth of the pockets around each tooth, as well as information on the presence of inflammation, bleeding, and other signs of periodontal disease. Only a doctor can fill in patient’s charting, but the staff can see the chart.

Dental Chart


Dental chart is a record of the health of a person's teeth and gums. It is used by dentists and dental hygienists to track and monitor the conditions of the patient's teeth, gums, and surrounding tissues. The chart includes information about the location and type of any cavities, gum disease, missing teeth, and other dental problems. In simpler terms, a dental chart is like a map of a person's mouth, showing the health of their teeth and gums. After recording problems, dentists can then pick a treatment to address that problem. Treatments recorded in one dental chart will form a treatment plan to be followed by the dentist to fix all of the patient’s dental issues.

Treatment Plan and Procedure


Treatment planning is necessary to keep track of what treatment your patient need. We encourage creating treatment plan so treatment process can be easily followed along by the dentist and the patient. In CMS, we make creating treatment plan easier just by filling dental chart, merging together the diagnosis and treatment decision and arrange it as a treatment plan. Each treatment plan will have the procedures and clinical notes attached to each treatment, to ensure the clinical records are easy to access and arranged neatly.
