Hover over More... and select Forms.

Click Create Form.

Fill out the form per your needs:

More questions can be added if you click on +Add Question.

Once completed, simply click Create Form.

Want to print your doc?
This is not the way.

Try clicking the ⋯ next to your doc name or using a keyboard shortcut (

CtrlP

) instead.
