Social Security System (SSS)

1. Accomplish Employer Registration (SS Form R1) in 3 copies, and must be signed originally by the authorized signatory. The start of operation is based on the date of employment of the first hired employee. Nature of business based on BIR 2303 (PSIC). The number of reported employees, based on the accomplished SS Form R1-A.

2. Accomplish Employment Report (SS Form R1A) in 3 copies. Fill up the complete details.

3. Accomplish Specimen Signature Card (SS Form L-501) in 2 copies. The person granting authority must be either the President, Chairman, or Corporate Secretary, and the appointed signatories must be either one of the incorporators or one of the reported employees per the Employment Report.

4. Submit original and photocopy of SEC-approved Articles of Incorporation, By-Laws.

5. Authorized signatories in all forms must be the President, Chairman, or Corporate Secretary whose name appears as one of the incorporators in Article 5 of SEC Article of Incorporation.

6. Original and photocopy of 2 valid IDs of signatories.

7. Submit a copy of the GIS, if available.

8. Secretary Certificate (for authorized representative).
