Log into then follow the instructions below. Go to AMENDMENTS on the top and click "Request Amendments" Click the circle next to “Notice of Association - Form 812” Enter the brokerage number to be associated with (950137-91)

Pay the $10.00 transfer fee. Take a screenshot of the payment confirmation page and then do one of the following: Send the screenshot of the payment confirmation page to: with the subject "%contact_first_name% %contact_last_name%, (your board)" The board would be the one you are currently with or plan to join within the next 30 days.
