pain

iianm, after the patient legally dies (and so probably isn't conscious), Alcor gives aesthetic meds that would make a living patient unconscious (usually used for some surgeries)
once your body is at liquid nitrogen temperature, for all practical purposes, there are no more chemical reaction going on in your body (so it seems to me no more subjective experience)

life insurance

you can sign up using "standard Life insurance", but needs to be with a company that has a high rating (at least an A- A.M. Best rating; source: ); you can ask any life insurance agent or used one Alcor recommends on their website

speed

in terms of how quick, the most important is the first phase given chemical reactions happen exponentially faster with temperature. there are often long delays during preservations, but during a high risk surgery, i think Alcor would prepare in some capacity. there are multiple case reports on Alcor's website. iirc, this contain some of the best cases:
