In this section, we focus on CFH’s vaccination efforts within Congregate Care Settings, encompassing Skilled Nursing Facilities, Long-Term Care Facilities, and Board and Care Homes. CFH’s mission extends to providing equitable access to healthcare services for the vulnerable populations residing in these settings. With a dedicated approach, CFH engages in outreach, coordination, and monitoring to ensure that these individuals receive the necessary vaccinations. This section is structured to shed light on our target population, the roles of CFH’s Homebound and Long-Term Care Coordinators, CFH’s Screening and Eligibility protocols, and our Tracking and Reporting mechanisms. Together, these components form the backbone of our commitment to safeguarding the health and well-being of residents in Congregate Care Settings.
