Section 3.2: Homebound Coordinators and Team

CFH has a dedicated team responsible for overseeing the coordination and implementation of vaccination services within congregate care settings. This team comprises two key members: the Homebound and Long Term Care Coordinator and the Homebound and Long Term Care Assistant Coordinator. Their roles are crucial in ensuring the success of our vaccine implementation plan tailored to congregate care clinic settings.

Section 3.2.1: Homebound Coordinators

Responsibilities of the Homebound and Long Term Care Coordinator and Assistant Include:
Outreach and Relationship Management: Our coordinators proactively engage with partners in congregate care settings, maintaining strong and collaborative relationships. They serve as the primary point of contact, fostering trust and effective communication.
Clinic Logistics and Site Needs: Working closely with key facility staff, our team confirms clinic logistics and identifies site-specific needs. This collaborative approach ensures that each clinic operates smoothly and meets the unique requirements of the facility.
Monthly Progress Report (MPR): The coordinators are responsible for compiling and reporting data for the Monthly Progress Report (MPR). They meticulously document vaccination activities, ensuring accurate and timely reporting to meet program requirements.
Vaccine Eligibility Verification: Using the California Immunization Registry (CAIR2), our team verifies the vaccine eligibility of individuals scheduled for vaccination. This step is essential to ensure that the right vaccines are administered to eligible recipients.
Insurance Coverage Verification: Our coordinators also check insurance coverage through Office Ally, ensuring that the vaccination process is seamless and accessible to all eligible individuals, regardless of their insurance status.
Connecteam Integration: The coordinators add congregate care clinics to our Connecteam platform, streamlining scheduling and communication with clinic staff. This digital integration enhances efficiency and coordination.
Roster Creation: Prior to each clinic, our team compiles a comprehensive roster of screened patients. This roster serves as a valuable resource for both the facility and the CFH clinical team assigned to the clinic, ensuring that all eligible individuals receive the appropriate vaccinations.
The Homebound and Long Term Care Coordinators play a pivotal role in the success of our vaccination efforts within congregate care settings. Their dedication, attention to detail, and strong partnership with facilities and healthcare providers ensure that we deliver equitable, accessible, and efficient vaccination services to those in need within these communities

Section 3.2.2: Data Analyst

Our Data Analyst is instrumental in our efforts to gather and analyze data from our monthly progress reports.
They provide valuable insights into our successes, helping us identify areas of focus and partners with whom we can collaborate more effectively.

Section 3.2.3: HPI Quartile 1 and 2 Outreach Coordinators

Our HPI 1 and 2 Outreach Coordinators focus on identifying potential partners and events within their respective HPI quartiles.
While their primary task is to locate partners for the specific clinics we report on, they also seek out strategic congregate care partners.
Once they establish a connection with potential partners, they pass on the referral to the Homebound Coordinator to initiate the next steps.
