Place a check-mark in the corresponding box each time you commit the error.
Determine which trading error is outpacing the rest and kill that Demon as quickly as possible and then move on to the next.
If you suffer the same Demon 8 times in a row, you may have some serious psychological issues that are blocking your progress. You should seriously consider whether trading is for you.
