Given Nick's feedback on the website refresh and that Auto pages should be more 'Book Online' this is my preference. However it should be reversed e.g.

For Glazing I'd stay with the original layout as it is above, although we are not using the form right now so should be call only (for the short term).
And for finding a branch I prefer this one, as it takes the customer to the locations page which makes sense from a customer journey perspective.
