Step 1: Fill out Form DS-11 to this link head.
Make sure to fill out all sections accurately, but do not sign the form until instructed to do so in the presence of a passport acceptance agent.

Step 2: Gather Required Documents
You will need to provide the following:
Proof of U.S. citizenship (such as a previously issued U.S. passport, birth certificate, or naturalization certificate)
Proof of identity (such as a valid driver's license, government ID, or military ID)
A photocopy of the front and back of the proof of identity
One passport photo (2x2 inches in size, taken within the last 6 months)

Step 3: Make an Appointment
Make an appointment at a passport acceptance facility as soon as possible.

Step 4: Submit the Application
At the appointment, submit the completed DS-11 form, the required documents, and pay the application fee.
The fee can be paid by check or money order, payable to "U.S. Department of State," or by credit/debit card.
There is also an execution fee that must be paid separately to the passport acceptance facility.

Step 5: Expedite the Application (if necessary)
If it's less than three weeks until your travel date and you haven't received your passport, you can request expedited service.
This will require an additional fee and proof of travel within 72 hours or urgent need of a passport.
