Request Consent

The system encourages a digital consent through an email address but allows for a scanned document to be uploaded for those not wanting to give consent digitally.

Where there is joint parental responsibility a second email address will be displayed.

If the user is not a Trusted Practitioner a confirmation dialogue will be displayed to state that until the Organisation Admin has approved the creation of the case, consent will not be requested.

The next stage of the process is to review the patient details provided by the parent.

Email Confirmations

After the case has successfully been created an Email will be sent to you as the Case initiator to confirm that the case consent email has been sent.

Subject: NDP Consent requested for a new case

Body: This is to notify you as the case initiator that consent has been requested for your new Neurodevelopmental Pathway case. There is a parent email added to the system.
