Section 5.2: Community Events Team

Community-Based Team: Bringing Clinics to Life
Our community clinics are made possible by the dedicated efforts of our community-based team, each playing a vital role in ensuring the success of these vital healthcare initiatives.

Section 5.2.1: Community Events Coordinator:

Our Community Events Coordinator serves as the primary liaison for partners who have completed our Immunization Request Form.
Responsible for overseeing the community sector, they engage with partners representing various types of locations, as well as those that may not fit typical clinic molds.
Their responsibilities include understanding partner needs, conducting site visits to assess event logistics, and confirming event details.
Once event details are confirmed, they work closely with our Scheduling Coordinators to ensure the event is added to our scheduling systems and public-facing calendars.
The Community Events Coordinator is also actively involved in partner meetings, referrals to other sector leads, and weekly meetings with event coordinators to review the upcoming schedule.
Additionally, they contribute to the completion of our monthly progress reports, providing essential insights.

Section 5.2.2: HPI 1 and 2 Outreach Coordinators:

Our HPI 1 and 2 Outreach Coordinators focus on identifying potential partners and events within their respective HPI quartiles.
While their primary task is to locate partners for the specific clinics we report on, they also seek out strategic community-facing partners.
Once they establish a connection with potential partners, they pass on the referral to the Community Events Coordinator to initiate the next steps.

Section 5.2.3: Community Health Workers:

Our dedicated Community Health Workers play a pivotal role in supporting our clinics.
They are not only responsible for fielding new location opportunities but also actively participate in clinics by engaging with the community.
They provide education, assist patients in completing registration forms, and serve as community mirrors, fostering a welcoming environment at our clinics.

Section 5.2.4: Data Analyst:

Our Data Analyst is instrumental in our efforts to gather and analyze data from our monthly progress reports.
They provide valuable insights into our successes, helping us identify areas of focus and partners with whom we can collaborate more effectively.
Together, this dynamic team ensures that our community clinics are not just events but integral components of our commitment to public health. Their dedication and collaboration enable us to reach diverse populations, provide vital healthcare services, and promote well-being within our communities.
