Section 5.1: Target Population and Outreach Strategies
Section 5.1.1: Target Population
Our vaccination outreach extends not just to community members but diverse communities, including congregations, refugees, migrant workers, and other high-risk subpopulations. Our aim is to ensure equitable access to vaccinations for everyone, with a particular focus on populations of focus outlined in our contract:
Black or African American population
Hispanic and Latinx population
Asian American, Native Hawaiian, or Other Pacific Islander population
American Indian or Alaska Native population
Individuals who reside in rural communities
Individuals who are homebound
Individuals who live in congregate care settings
We prioritize our efforts in areas designated as HPI (Healthy Places Index) quartiles 1 and 2, where health disparities and socioeconomic challenges are more pronounced. In this section, we will explore our strategies for identifying and reaching these diverse populations, ensuring that no barriers stand in the way of their access to life-saving vaccinations.
Healthcare Disparities: We recognize and address healthcare disparities that disproportionately affect the Black or African American population.
Culturally Competent Care: Our healthcare providers undergo cultural competence training to provide respectful and culturally sensitive care.
2. Hispanic and Latinx Population:
Linguistic Access: Bilingual staff and interpreters are available to ensure effective communication with Hispanic and Latinx individuals.
Trusted Partnerships: We collaborate with community organizations that serve this population to build trust and increase vaccination rates.
3. Asian American, Native Hawaiian, or Other Pacific Islander Population:
Multilingual Outreach: We offer multilingual materials and services to engage with diverse linguistic communities within this population.
Culturally Sensitive Care: Our vaccination teams are trained to understand and respect cultural nuances.
4. American Indian or Alaska Native Population:
Collaboration with Tribal Entities: We are working towards increasing partnerships with tribal entities to provide culturally relevant healthcare services.
Accessibility: We are improving our outreach strategies so that vaccination clinics can be easily accessible to members of this population, including those in remote areas.
5. Individuals Who Reside in Rural Communities:
Mobile Clinics: We deploy mobile vaccination clinics to rural communities, overcoming geographical barriers.
Outreach and Education: Our outreach efforts focus on raising awareness about the importance of vaccination in rural areas.
Subpopulations:
Within these priority populations, we also recognize the existence of subpopulations with unique healthcare needs. These may include:
Older Adults
Children and Adolescents
Pregnant Individuals
Individuals with Underlying Health Conditions
LGBTQ+ Community
For each subpopulation, we tailor our outreach strategies and vaccination services to address their specific healthcare requirements. Our goal is to ensure that no community is left behind, and we continuously adapt our approach to meet the evolving needs of our diverse community members.
Section 5.1.2.2: HPI Quartiles 1 and 2:
HPI Quartiles 1 and 2: Addressing Health Disparities
CFH places a significant emphasis on reaching communities designated as HPI (Healthy Places Index) quartiles 1 and 2. These quartiles represent neighborhoods facing higher health disparities and socioeconomic challenges. Our commitment to equity and accessibility drives our approach to outreach in these areas, ensuring that we address health disparities effectively.
Outreach Strategies:
Community Engagement: We actively engage with residents and community leaders in HPI quartiles 1 and 2. This includes participating in community meetings, town halls, and events to understand their specific healthcare needs and concerns.
Culturally Tailored Outreach: Our outreach materials and strategies are culturally sensitive and tailored to the diverse populations within these quartiles. We understand the importance of connecting with individuals in a way that resonates with their cultural backgrounds.
Language Access: Bilingual staff and interpreters are available to facilitate communication with non-English-speaking residents, ensuring they have access to vital health information.
Collaboration with Community-Based Organizations: We partner with local community-based organizations that have established trust within these communities. These partnerships help us leverage existing networks to expand our outreach efforts.
Mobile Clinics: To overcome transportation barriers, we deploy mobile vaccination clinics to easily accessible locations within these quartiles.
Examples of Impactful Outreach:
Faith-Based Institutions: We collaborate with local churches, mosques, and temples in HPI quartiles 1 and 2 to host vaccination clinics. These institutions often serve as trusted community hubs where residents feel comfortable receiving healthcare services.
Grocery Store Partnerships: By partnering with neighborhood grocery stores, we bring vaccination services closer to residents' daily routines, increasing accessibility.
Community Health Fairs: We actively participate in community health fairs and events held in these quartiles, providing vaccinations as part of a comprehensive health and wellness offering.
Our approach goes beyond simply providing vaccinations; it involves building strong connections with the residents of HPI quartiles 1 and 2. We believe that by addressing health disparities and improving access to healthcare in these underserved areas, we contribute to the overall well-being and health equity of our community.
By leveraging culturally sensitive outreach, trusted partnerships, and accessible vaccination services, we aim to make a lasting impact in HPI quartiles 1 and 2, ensuring that everyone has an equal opportunity to receive essential vaccinations.
Section 5.1.2.3: Hosting Sites at Trusted Locations:
CFH’s commitment to reaching all segments of the population extends to the locations where we host vaccination clinics. We understand that accessibility and comfort are crucial factors for individuals when deciding to receive healthcare services. To this end, we actively seek out and establish vaccination clinics at trusted locations within the community, even if they may be unconventional for a standard clinic setting.
Collaboration with Local Leaders:
We collaborate closely with local community leaders, religious leaders, community organizers, and business owners. These individuals often have a deep understanding of the community's needs and preferences.
Through conversations and partnerships with these local leaders, we identify trusted locations where residents are more likely to feel comfortable and safe receiving vaccinations.
Unconventional but Trusted Settings:
Our approach goes beyond traditional healthcare facilities. We are open to hosting clinics in unconventional settings such as community centers, places of worship, schools, grocery stores, parks, and even local businesses.
By choosing these trusted locations, we ensure that residents have easy access to vaccinations in familiar and convenient environments.
Community Buy-In:
Hosting clinics at trusted locations garners community buy-in and participation. Residents are more likely to attend a vaccination clinic when it's held at a place they frequent or trust.
This approach fosters a sense of community ownership and engagement, leading to higher vaccination rates and overall health awareness.
Customized Outreach:
Our outreach strategies for each trusted location are tailored to the specific needs and preferences of the community it serves.
We work with local leaders to engage residents through culturally sensitive and community-specific outreach materials and messages.
Examples of Trusted Locations:
Place of Worship Clinics: Working with religious leaders, we set up vaccination clinics at churches, mosques, temples, and synagogues.
Community Center Clinics: Collaborating with county partners, we host clinics at target County Public Health Center, where residents can receive their vaccines outside of regular business hours.
Business Partnerships: Partnering with local businesses, we establish clinics in areas with high foot traffic such Northgate Gonzalez Markets, making vaccinations easily accessible during daily routines.
Our commitment to hosting clinics at trusted locations reflects our dedication to ensuring that everyone in the community has equitable access to vaccination services. By prioritizing comfort, accessibility, and community engagement, we aim to break down barriers and make vaccinations a seamless part of community life.
Section 5.1.2.4: Hosting Clinics Outside of Normal Business Hours:
CFH is committed to ensuring that vaccination clinics are not only accessible but also convenient for all members of the community. We recognize that people have diverse schedules and preferences, and offering services outside regular business hours is essential to reach the widest possible audience.
Weekend Clinics:
In collaboration with the County of San Diego Public Health Centers, we have extended our services to weekends. This initiative allows individuals who may have weekday commitments to access vaccinations during their free time.
Weekend clinics are strategically placed to maximize convenience, making it easier for community members to prioritize their health.
Tailoring Clinic Hours:
We understand that different sites have varying needs and schedules. Therefore, we tailor our clinic hours to align with the specific requirements of our partner sites.
For example, when partnering with farms or nurseries, we often schedule early morning clinics to accommodate the work routines of their teams. This ensures that employees can receive vaccinations without disrupting their daily tasks.
Conversely, we also offer evening clinics, particularly during health fairs or alongside events in strategic locations such as nightclubs.
Community-Centric Timing:
To determine the optimal clinic hours, we engage with our partner sites and the local community to identify when foot traffic is highest and when people are most likely to attend.
By aligning our clinic hours with the needs and preferences of the community, we ensure that vaccination services are both accessible and appealing.
Strategic Partnerships:
Collaboration with strategic partners, such as nightclubs like The Rail, allows us to reach individuals in settings where they feel comfortable and are already present.
By hosting clinics during events or at venues where community members gather, we break down barriers to vaccination by integrating healthcare services into existing social activities.
Our commitment to hosting clinics outside of normal business hours reflects our dedication to removing barriers to vaccination. By offering flexibility in timing and tailoring clinic hours to suit the needs of diverse sites and communities, we aim to make vaccination a convenient and easily accessible option for everyone.
Section 5.1.2.5: Partnerships and Collaborations:
CFH firmly believes that successful vaccination outreach requires strong partnerships and collaborations within the local community. By working hand-in-hand with local organizations and entities, we enhance our ability to reach underserved populations and ensure that vaccinations are accessible to all.
Community-Facing Collaboratives:
We actively participate in various strategic community-facing collaboratives that bring together local leaders, community-based organizations (CBOs), businesses, health plans, county representatives, and more. These collaboratives serve as platforms for networking, event promotion, and collaborative planning.
Through these collaboratives, we have the opportunity to engage with key stakeholders, share information about our vaccination events, and gain insights into potential events that align with our mission.
These partnerships have yielded population-specific initiatives, exemplified by our collaboration with the Live Well on Wheels team.
Rural Flu Tour:
Our partnership with the Live Well on Wheels team resulted in the launch of the Rural Flu Tour. This initiative focuses on identifying rural zip codes with limited access to healthcare services.
We schedule vaccination clinics at convenient locations within these rural communities, such as libraries, schools, or community centers. The Live Well on Wheels bus accompanies us, providing additional resources and support.
The Rural Flu Tour exemplifies our commitment to reaching underserved and remote populations, ensuring that everyone has equal access to vital vaccinations.
Community Engagement:
Beyond collaboratives, we actively engage with the community by participating in events at community gardens and partnering with organizations that have received funding to distribute gift cards as incentives for vaccination.
These collaborations demonstrate our dedication to meeting community members where they are, making vaccinations accessible in familiar and comfortable settings.
Our partnerships and collaborations are central to our mission of achieving widespread vaccination coverage. By fostering relationships with local organizations and actively participating in community initiatives, we strengthen our ability to reach diverse populations and address healthcare disparities.
Section 5.1.2.6: Pairing Clinics to Promote County or National Health Initiatives:
CFH is dedicated to aligning our vaccination clinics with county and national health initiatives, actively contributing to the promotion of public health goals. By partnering with these initiatives, we ensure that our efforts are synchronized with broader healthcare objectives.
Collaboration with Love Your Heart Initiative:
A prominent example of our commitment to aligning with health initiatives is our collaboration with the County of San Diego's Love Your Heart Initiative. This countywide, now statewide, initiative encourages community members to prioritize their cardiovascular health by receiving free blood pressure screenings at designated Love Your Heart locations.
As a longstanding partner of Love Your Heart, we strategically pair our clinic services with participating Love Your Heart locations. Our clinical team, comprised of trained medical volunteers, not only administers vaccinations but also offers blood pressure checks.
This partnership goes beyond the scope of vaccination, contributing to the overall health and well-being of the community.
Engagement in Bi-National Health Week:
Another significant partnership involves our active engagement in the county's Bi-National Health Week, an initiative aimed at fostering health and unity among cross-border partners.
We collaborate with Ventnilla De La Salud, an organization that coordinates events for Bi-National Health Week, including activities in conjunction with the Mexican consulate.
By participating in these events, we extend our reach to diverse populations and strengthen relationships with our cross-border partners.
These examples underscore our dedication to the broader public health landscape. By pairing our clinics with county and national health initiatives, we amplify our impact and contribute to the promotion of essential health services. Our commitment to public health extends beyond vaccination, encompassing a holistic approach to community well-being.
