Effective screening and eligibility criteria are fundamental to ensuring that homebound patients receive vaccinations tailored to their specific needs. Our process is designed to identify individuals who are at higher risk for exposure, infection, hospitalization, and mortality from COVID-19 and other vaccine-preventable diseases. Eligibility Criteria:
Our eligibility criteria are in alignment with the contract requirements, which focus on administering vaccines to individuals who are medically underserved and belong to specific demographic groups at higher risk. These groups include:
Black or African American population
Hispanic and Latinx population
Asian American, Native Hawaiian, or Other Pacific Islander population
American Indian or Alaska Native population
Individuals who reside in rural communities
Individuals who are homebound
Individuals who live in congregate care settings
Referral List Verification:
Upon receiving the referral list from the County's Epidemiology and Immunizations Services Branch (EISB) team, our Homebound Coordinators carefully verify each patient's eligibility against the established criteria. This verification process ensures that the right individuals are identified and prioritized for vaccination services.
Insurance Coverage and Vaccine Eligibility:
In addition to eligibility criteria, we cross-reference patient information with the California Immunization Registry (CAIR2) to confirm vaccine eligibility. We also check insurance coverage through Office Ally. This dual-check approach ensures that the appropriate vaccines are administered, and, if applicable, insurance billing is processed accurately.
