Outreach Strategy:
Unlike our usual outreach strategies, our approach to reaching homebound patients is a collaborative effort with the County's Epidemiology and Immunizations Services Branch (EISB). This partnership streamlines the process of identifying and connecting with homebound individuals who require in-home vaccination services.
Referral Process:
The County's EISB team receives referrals through multiple channels, with the primary source being the county's 2-1-1 resource line. This resource allows patients to self-identify as homebound individuals seeking in-home vaccination services for themselves or their loved ones. Upon receiving these requests, the EISB team promptly contacts the patients to gather essential information.
Generation of Referral List:
Once the necessary information is collected, the County's EISB team generates a referral list. This list is then shared with our dedicated homebound coordinators, who are responsible for following up with the patients and scheduling vaccination appointments.
Equity and Accessibility:
Our commitment to equity and accessibility extends to homebound individuals. By collaborating with the County's EISB, we ensure that those who are homebound, especially those from underserved communities, have equal access to vital vaccination services.
