Program Overview

According to the 2016 Community Health Needs Assessment (CHNA)1, while conducting community engagement activities there were 10 key social determinants of health identified. Access to Care or Services included barriers to access which listed transportation and language as critical. Further, a recent study of trends and implications for healthcare access in relation to increased transportation barriers during COVID-19 reports a compounding inequity stating “The same disadvantaged groups (especially Black, Indigenous, and people/communities of color and low-income people) who face elevated risks for many health conditions are also more likely to experience the transportation barriers that prevent them from accessing care. In addition, these groups are most threatened by both the health and economic impacts of COVID-19. These intersecting disparities converge to continuously over-burden the same communities”.2 ​​​​​​​​​​​​​​The report concludes that there are several interventions and policy approaches that can help mitigate these transportation barriers. CHF responds to this call to action by being a part of the solution, using “novel funding mechanisms, and flexible and creative service delivery solutions.”
CFH’s Immunize San Diego program partners with churches, schools, community centers, and volunteer nurses to provide free immunizations to low-income individuals. CFH has concentrated our preventive and specialty care efforts in Healthy Places Index (HPI) zip codes even before they were termed so. We have the right mix of eager medical, healthcare, and community partners to fill the COVID-19 vaccination need. Our successful approach combines community partners serving their community with a client base who trusts them. With our efficient process and friendly compassionate volunteers, we have a recipe for success during a potentially stressful process.
CHF strives to be a pillar of reliability, strength, and equity among health partners in San Diego. We address diversity, racial equity, and inclusion through a data-driven lens, taking into consideration the social determinants of health such as distance of services from patient's homes and clinic partners, identifying community wellness disparity or access hot spots using indicators including race, ethnicity, language, and other cultural barriers.
