Section 1.1: Roles and Responsibilities of School-Based Coordinators:
Champions for Health (CFH) brings its wealth of experience and expertise in coordinating, planning, and providing healthcare services to the dynamic environment of school-based clinics. The effective coordination, planning, and execution of these clinics are spearheaded by a dedicated team that reports directly to the Director of Community Health. This section delves into the multifaceted roles and responsibilities of our School-Based Coordinators, each with a specific focus and mission within the broader framework of our School-Based Coordinator Program.
Section 1.1.1: Clinical School Coordinator (CSC)
The Clinical School Coordinator position plays a pivotal role in bridging the gap between healthcare providers and the school community. Filled by a Registered Nurse (RN), the CSC serves as the primary liaison between the School Workforce Grant HHSA Public Health Nurses (HHSA PHNs), District Nurses, and CFH staff. Their responsibilities encompass a range of critical tasks, including:
Collaborating with HHSA PHNs to understand the objectives of the County School Workforce Grant and accessing data on under-vaccinated schools.
Establishing connections with district nurses and participating in school nurse workgroup meetings to identify immunization needs.
Learning the district nurses' assessment processes for student vaccination and health insurance status.
Setting clear clinic expectations with school administrators and verifying assessment reports using our CAIR2 team to ensure vaccine eligibility.
The CSC also collaborates closely with the School Events Coordinator to ensure all necessary site contacts, materials, and vaccines are readily available. Additionally, they work hand-in-hand with the Vaccine for Children Coordinator to maintain vaccine inventory, train clinical and Community Health Worker (CHW) staff, and ensure compliance with Vaccines for Children (VFC) protocols.
Section 1.1.2: School Event Coordinator (SEC)
The School Event Coordinator (SEC) is the linchpin in the successful coordination of school-based events. Serving as the primary point of contact for school sites, the SEC is tasked with a diverse set of responsibilities:
Gathering crucial information, such as main points of contact, site requirements (e.g., materials, vaccines), and event scheduling details.
Conducting site visits to ensure all logistical needs are met and attending parent or administrator staff meetings when necessary.
Maintaining regular communication with school sites to guarantee a seamless process and checking in at least one week before scheduled events.
Collaborating with Public Health Nurses (PHNs) to coordinate vaccine presentation requests and capture presentation data to share with school sites.
Partnering with Community Health Workers (CHWs) to organize canvassing activities, ensuring both population and site-specific details are well-prepared.
Working alongside the Vaccine for Children Coordinator to check vaccine inventory, receive up-to-date training on various vaccines, and ensure all partners are well-versed in California Department of Public Health's Vaccine Eligibility guidelines.
The SEC also actively participates in partner meetings within the education sector, advocating for the importance of childhood vaccines and their availability.
Section 1.1.3: Vaccine for Children Consultant (VFCC)
The Vaccine for Children Consultant (VFCC) plays a pivotal role in ensuring CFH's compliance with Vaccines for Children (VFC) protocols and the oversight of VFC-funded vaccines. Their responsibilities encompass:
Supporting the creation of VFC-specific screening tools and requesting informational material from the State.
Monitoring vaccine inventory and ensuring accurate records of VFC vaccine doses administered, wasted, and in inventory.
Collaborating with the Quality Assurance team to ensure compliance with VFC program guidelines and regulations.
Developing School-Based Clinic VFC Eligibility Verification protocols to ensure that only VFC-eligible youth receive the VFC-funded vaccines.
Coordinating with the Clinical School Coordinator (CSC) and Clinical Training Coordinator to create training modules on VFC vaccine management, preparation, administration techniques for pediatric patients, and VFC program reporting requirements.
This role is instrumental in maintaining the highest standards of vaccine administration and ensuring that VFC-eligible children receive the vaccines they need.
Section 1.1.4: Early Childhood Clinic Coordinator (ECCC)
The Early Childhood Clinic Coordinator (ECCC) focuses on school-based clinics, with a specific emphasis on Pre-K and early education partners. Their role is indispensable in ensuring the smooth operation of immunization programs in these settings. Key responsibilities include:
Serving as the primary liaison between CFH and educational institutions, gathering essential information about site requirements, materials, vaccines, and event scheduling.
Conducting site visits, attending relevant meetings, and maintaining regular communication with early education partners.
Active participation in partner meetings within the education sector, advocating for the importance of childhood vaccines and their availability.
Collaboration with Public Health Nurses (PHNs) to coordinate vaccine presentation requests and share critical data with school sites.

Our HPI 1 and 2 Outreach Coordinators are instrumental in identifying potential partners and events within their respective HPI quartiles. While their primary task is to locate partners for specific clinics, they also seek out strategic school-facing partners. Once they establish connections with potential partners, they pass on referrals to the School Events Coordinator to initiate the next steps.
Section 1.1.6: Vaccine for Children Vaccination Data Coordinator (VFCVDC)
The VFC Vaccination Data Coordinator plays a crucial role in tracking and reporting Vaccines for Children (VFC) clinics efficiently. Their responsibilities include:
Tracking and recording all VFC vaccine doses administered, with detailed documentation for each vaccine type.
Monitoring and reporting on vaccine wastage, identifying areas for improvement in vaccine handling and administration.
Maintaining up-to-date inventory records of VFC vaccines, coordinating with the Vaccine for Children Coordinator to ensure an adequate supply.
Generating reports and data analysis related to VFC clinics, supporting program evaluation and decision-making.
Collaborating with the Clinical School Nurse Coordinator and other team members to ensure accurate documentation of immunizations in school-based clinics.
Participating in VFC program training to stay current on vaccine eligibility guidelines and program requirements.

Compiling and organizing data from monthly progress reports.
Analyzing the data to identify trends, successes, and areas for improvement.
Generating reports and presentations to share findings with the team.
