Lorain County Public Health (LCPH) is the local public health agency serving Lorain County, Ohio. We provide public health services to all communities within Lorain County, except the City of Vermilion, which is served by the Erie County Health Department. Through prevention, education, partnerships, and public health services, LCPH works to improve the health and well-being of our community and remains committed to advancing maternal and child health outcomes. Through this funding opportunity, LCPH seeks to expand access to maternal behavioral health peer support services for pregnant and postpartum women across Lorain County.

Why This Funding Matters

Ohio's infant mortality goal is to reduce infant deaths to 4.4 or fewer per 1,000 live births. Infant mortality is widely recognized as an indicator of community health, reflecting factors such as access to healthcare, poverty, socioeconomic conditions, and the overall well-being of children and families.
In 2023, Lorain County's infant mortality rate was 6.1 per 1,000 live births. While lower than Ohio's statewide rate of 7.1 per 1,000 live births, opportunities remain to improve outcomes for mothers, infants, and families throughout the county.
Addressing the factors that influence maternal and infant health requires collaboration across healthcare providers, community organizations, and support systems serving families.

Funding Opportunity Overview

Lorain County Public Health (LCPH) is eligible to receive funding from the Ohio Department of Children and Youth (DCY) to support Maternal Behavioral Health Peer Support Programming in Lorain County.
Through this Request for Proposals, LCPH is seeking qualified organizations to implement peer support services that help pregnant and postpartum women improve their mental health wellness and access community-based support resources.
Eligible organizations serving Lorain County may apply for up to $58,334 for the period of September 1, 2026 through June 30, 2027, contingent upon LCPH's receipt of funding from DCY and execution of a fully signed subrecipient agreement.

SCOPE OF WORK & REQUIRED SERVICES

outreach

Outreach & Enrollment

Conduct outreach and education to promote peer support services.
Foster collaborative partnerships to identify and refer potential participants.
Screen, enroll, and serve individuals with needs that align with peer support services.
Provide confidential space for screenings and enrollments.
Establish and implement intake process for individuals referred for services.
Complete at least 50 screenings per quarter using an evidence-based screening tool (e.g., PHQ-9 or Edinburgh).
Facilitate referrals (warm handoffs) for participants who need a different level of support.

Peer Support Services

Provide individual and/or group peer support sessions.
Provide confidential space to conduct individual and group sessions.
Facilitate group and/or individual sessions for participants recommended for peer support services.
Conduct services in-person or virtually.
Maintain a log of all participant incentives and enablers purchased and distributed.
Provide referrals to collaborative partners for needs outside of peer support services.
Refer participants who need a different form of support to the appropriate referral source.
Attempt outreach three times for individuals who have been enrolled and attended at least on session.

PROGRAM OPERATIONS

Staffing & Supervision

Comply with Ohio Department of Behavioral Health requirements regarding peer support supervision as well as Ohio Revised Code Section 340.032 and Ohio Administrative Code Rule S122-29-15.
Ensure peer supporters comply with Ohio Department of Behavioral Health supervision requirements.
Require peer supporters to attend maternal mental health substance abuse, peer support model, and trauma informed care trainings, as appropriate.
Participate in technical assistance sessions with LCPH.

Monthly Reporting Requirements

Create forms and a tracking system so information can be reported to DCY. Reports should only refl ect individuals who are pregnant and up to one year postpartum. Report this data monthly, by the 10th of the following month:
Referred for peer support services
Screened for peer support services
Enrolled (i.e., newly and previously enrolled), by race and ethnicity
Newly enrolled (i.e., newly enrolled during reporting month), by race and ethnicity.
Unduplicated individuals who attended at least one individual session
Unduplicated individuals who attended at least one group session.
Referred for a different level of support (i.e., warm handoff to a different mental health provider).
Group sessions scheduled.
Group sessions conducted
Individual sessions scheduled
Individual sessions conducted
Participants who graduated from peer support services
Peer support waitlist.

Scoring Method

Proposals will be reviewed and scored using a standardized evaluation rubric. Each criterion is scored on a scale of 0–2 points:
2 Points – Exemplary: Fully addresses the criterion and demonstrates strong alignment with program goals.
1 Point – Sufficient: Adequately addresses the criterion but may have minor gaps or limitations.
0 Points – Inadequate: Does not sufficiently address the criterion or lacks required information.
Applications demonstrating strong alignment with community needs, grant requirements, and measurable outcomes will receive the highest scores.
