Using this template

This template includes a table and a form which can be customized to include any questions. Feel free to make it personal — have colleagues upload fun photos, or create a scale where they can indicate how much they miss you!

You can publish your form and embed it in any Coda doc, or send the link over to your team via email, Slack or other method.
With Coda forms, team members do not have to have a Coda account to submit answers, and they are unable to view any data from anyone else unless you choose to share this doc with them.
