When you first log in/create an account in the Portal you will see the image below.
In order to view your personal account make sure to click on the Account Icon on the top right corner.
From here you will be able to add new dependents by clicking on the “Household” option on the right side menu.
Select the “ADD A HOUSEHOLD MEMBER” button that is located near the top right.
Once you have clicked the button make sure to fill out the corresponding information for the dependent.
Once the information is complete make sure to hit the save button.
Fantastic! If you want to add more dependents then make sure to repeat the steps in the previous “
a New Dependent”
section of the document.
Now that you have added a new dependent you are able to sign the waiver. Proceed by selecting the “Sign
Waiver” option on the left side Menu.
Selecting this will lead you to the following screen. The red boxes are highlighting where you will select a
location and also where you will select the people that are signing for.
Once these choices are done make sure to select the blue “SIGN” button.
Selecting this button will take you to the following image. Here is where you can sign a waiver. Make sure to
initial where it is required by clicking on the field and to sign at the end.
Once all of the fields have been filled out make sure to push the final blue button labeled: “Agree to these
