Separation anxiety or chewing? are they crate trained? how long are they ok alone?
Escapist?
Affectionate and cuddly?
Do they have any training? What commands? Are they food motivated?
House trained? Any signals for going outside?
Where do they usually sleep?
Vocal?

Exercise and play
How much exercise is it currently getting/ how active?
How is recall?
Any leash issues? puling or reactivity?
Play preferences - chewing / fetch / not / puzzles

Health
Any health or dental issues? What veterinary care have they had recently?
Up to date on heart-worm and tick medication and vaccinations? Do we get a proof?
Any health problems?
microchipped? how do we get this updated?
neutered?
Food preferences/ amount / any restrictions?
How are they with grooming?
