Time Off Request

POLICY:

In general, PTO is primarily used for vacation time, and is available in the event of illness or injury, medical appointments and unexpected personal business only. To request vacation PTO, you must notify your supervisor for approval at least 3 weeks in advance. PTO can be requested in minimum increments of one full 8-hour day and maximum concurrent increments of 5 days. For reasons other than vacation time, such as sick or personal time, the employee must contact his or her supervisor before the start of their workday.

All PTO is paid at the employee's straight-time hourly or salary rate of 8 hours at the time of absence, except where noted below regarding 100% commissioned Sales reps. PTOs do not include overtime or any special forms of compensation such as incentives, commissions or bonuses.

Use It or Lose It Policy

Use It or Lose It Policy Management is an advocate of a balanced work-life, and we encourage all employees to use their allotted PTO days for such purpose. If an employee does not use all of their PTO days by the time of their next anniversary (date of hire), those PTOs will not carry over and will be lost. Management reserves the right to amend and suspend this policy at any given time for any reason.
