My Story

During a basketball game in my sophomore year of high school, I was charged and my head slammed into the hard wood court. I was diagnosed with a level 3 severe concussion, but neither my family nor I knew what to expect. It has been almost a year since my injury, and I now know so much more about what it means to live with a concussion: missing countless tests, stopping sports, constantly communicating with doctors and teachers, and negotiating academic accommodations, etc. This past year I have received so much support and knowledge, and I want to share all that I have learned to help others in their recovery process!

What is this for?

This is a document was created for the sole purpose of making concussion recovery as straightforward as possible.

Who is this for?

This document is for those who have suffered concussions as well as the people who are trying to support them. A lot of people who have suffered concussions aren’t able to look at screens, so many aspects of this doc are set up for the person filling it out on the patient’s behalf.

Getting Started!

When I got a concussion, I discovered that tracking daily events and symptoms was critical for my recovery. In the Health Log Overview, it will debrief you on what the log is and what it does.

Knowing my who was in support system helped make life with a concussion that much easier. This database of contact information will allow you to communicate with those you need to.

Communication was key to my recovery! I’ve included reference emails and some guidelines that helped me clearly communicate with my support system.

Easily track class specific accommodations, exempted work, and missed work. Having these written down made my life a whole lot easier!
