To accomplish this, CFH has established a screening and eligibility process that not only encompasses the Vaccine for Children (VFC) program but also extends to state-funded flu vaccines and general eligibility criteria for hosting a school-based clinic. Our screening and eligibility protocols are designed to identify those who are eligible for specific vaccines, verify documentation, and streamline the vaccination process. In this section, we will provide an overview of our comprehensive screening and eligibility procedures, emphasizing our commitment to delivering vaccines efficiently while adhering to all regulatory requirements. Section 1.4.1: Vaccine for Children (VFC) Screening and Eligibility

At Champions for Health, we employ a comprehensive approach to verify VFC eligibility, which includes collaboration with school administration and utilizing insurance eligibility checks through our clearinghouse, Office Alley.

Collaboration with School Administration

To streamline the verification process, CFH will work closely with school administration to identify students who meet VFC eligibility criteria. This collaboration involves cross-referencing our vaccine roster with the school's student roster to pinpoint eligible individuals. By leveraging the school's records, we can accurately identify and confirm VFC-eligible students, ensuring they receive the necessary vaccinations.

2. Insurance Eligibility Checks through Office Alley

In addition to school collaboration, CFH will utilize a trusted clearinghouse for insurance eligibility verification. This system allows us to check the insurance status of individuals to confirm their eligibility for the VFC program. By conducting insurance checks through Office Alley, we can further validate eligibility, ensuring that vaccines are provided to eligible recipients while maintaining compliance with VFC guidelines.

Section 1.4.2: State-Funded Flu Vaccine Eligibility

Our school-based clinic program extends its services to administer state-funded flu vaccines to eligible individuals, with a primary focus on school-aged children and their families. Eligibility criteria for receiving state-funded flu vaccines include being uninsured, underinsured, or an attendee at the scheduled school based clinic. At Champions for Health, we adhere to these criteria to ensure equitable distribution.

Section 1.4.3: General Eligibility for School-Based Clinics

Our commitment to providing school-based clinic services extends to all schools within our reach. While CFH aims to serve as many schools as possible, we prioritize those located within communities that may face greater health disparities. Specifically, we give priority to school districts situated within Health Places Index (HPI) quartiles 1 and 2, or those identified by the county as in need of additional support.

CFH’s primary focus is on promoting health equity and addressing healthcare disparities in underserved communities. As such, schools located in HPI quartiles 1 and 2 receive priority access to our services. These communities often face various health-related challenges, and CFH’s goal is to ensure they have equitable access to essential healthcare resources, including vaccinations.

Military Population Consideration

While CFH is committed to serving a wide range of schools, it's important to note that eligibility for state-funded vaccines, is subject to specific guidelines. According to the county's state flu program, individuals covered under military healthcare plans may not be eligible to receive state-funded flu vaccines. Therefore, when receiving a vaccine request from a school, we assess the composition of the student population.

Seeking Clarification

If CFH receives a vaccine request from a school that may not fall within the target area or has a predominantly military-affiliated student population, CFH follows a protocol of seeking clarification from the county's state flu program. This collaborative approach ensures that eligibility for state-funded vaccines is consistently and accurately determined, aligning with program guidelines.
CFH’s overarching goal is to provide accessible healthcare services to schools while adhering to eligibility criteria set forth by state programs. By doing so, we can effectively contribute to the health and well-being of students, their families, and the broader community.
