Contract 568312 - Champions for Health: School Based Coordinator Implementation Plan

Section 1.2: Partnerships with Schools:

Our commitment to equitable healthcare extends to the heart of our vaccination efforts, where we prioritize reaching communities designated as HPI (Healthy Places Index) quartiles 1 and 2. These quartiles serve as indicators of neighborhoods facing pronounced health disparities and socioeconomic challenges. To strategically address the unique needs of these communities, we employ a multi-faceted approach that leverages county school vaccination data, fosters collaboration with key school administration staff, engages in partnerships through meetings, and places a strong focus on a diverse population encompassing Black or African American, Hispanic and Latinx, Asian American, Native Hawaiian, or other Pacific Islander populations, as well as American Indian and Native Alaskan, and residents of rural communities. Our overarching goal is not only to vaccinate enrolled students but to extend our reach to their families, school staff, and the broader community, ultimately contributing to community immunity and enhancing overall well-being in these underserved neighborhoods.

Section 1.2.1: Targeting HPI Quartiles 1 and 2

To ensure that our vaccination efforts are reaching the communities most in need, we place a significant emphasis on targeting areas designated as HPI (Healthy Places Index) quartiles 1 and 2. These quartiles represent neighborhoods facing higher health disparities and socioeconomic challenges. Our approach to outreach in these areas is multi-faceted and strategically designed to maximize impact.

Section 1.2.2: Utilizing County School Vaccination Data

One approach is leveraging county school vaccination data, which provides valuable insights into school enrollment and vaccination coverage rates in different neighborhoods. By analyzing this data, we identify schools located within HPI quartiles 1 and 2. This information guides our initial selection of school sites for vaccination clinics.

Section 1.2.3: Collaborating with Key School Administration Staff

To establish a strong presence within these communities, we engage in close collaboration with key school administration staff, particularly district nurses. District nurses are invaluable partners who possess in-depth knowledge of their schools' communities and healthcare needs. We consult with them to pinpoint the schools with the greatest vaccination needs and work together to coordinate clinic logistics.

Section 1.2.4: Collaboration with Partners Through Meetings

Collaboration is a cornerstone of our approach. We regularly convene meetings with partner organizations, including PTA meetings, regional collaboratives, and education administration workgroups. These meetings serve as platforms for discussing outreach strategies, sharing data, and aligning efforts to target HPI quartiles 1 and 2 effectively.

Section 1.2.5: Diverse Population Focus

Our target population in these areas is diverse, encompassing Black or African American, Hispanic and Latinx, Asian American, Native Hawaiian, or other Pacific Islander populations, as well as American Indian and Native Alaskan, and rural communities. By recognizing the unique healthcare needs and cultural considerations of each group, we tailor our outreach and vaccination services to be culturally sensitive and accessible.

Section 1.2.6: Vaccinating Enrolled Students, Families, School Staff, and the Community

Our goal extends beyond vaccinating enrolled students; we aim to reach their families, school staff, and the broader community. By offering comprehensive vaccination services to these groups, we contribute to overall community immunity and foster a culture of health and well-being within these neighborhoods.
In summary, our targeted outreach efforts in HPI quartiles 1 and 2 involve a data-driven approach, collaboration with key stakeholders, and a focus on addressing health disparities among diverse populations. This comprehensive strategy ensures that we maximize the impact of our school-based clinics in underserved communities.
